What is bilirubin?
Bilirubin is a compound that forms as a normal part of the life cycle of red blood cells when aged or abnormal blood cells are broken down. Haemoglobin from red blood cells is separated into globin and haem, which is further broken down into bilirubin. Bilirubin is transported to the liver, where it is made water-soluble by conjugation with glucuronic acid so it can be excreted in bile.
Bilirubin that is conjugated with glucuronic acid is called direct bilirubin. Unconjugated bilirubin is called indirect bilirubin and is toxic in high concentrations. Bilirubin and its subsequent breakdown products are responsible for the distinctive yellow colour in bruising and brown in faeces. Symptoms associated with abnormal bilirubin production include jaundice, blocked bile ducts and liver problems.

This test measures the amount of direct bilirubin in the blood. It is usually used to diagnose liver problems and along with a total bilirubin test, can be used to estimate the amounts of direct and indirect bilirubin in the body.
